The Ministry of Health and Welfare (MOHW) today (April 13) held the "First Taiwan Top 50 SMART on FHIR Applications Award Ceremony and SMART SUMMIT International Forum" at the Taipei International Convention Center, selecting 50 applications with clinical value and development potential from 115 submissions — hailed as the "ETF 0050" of healthcare technology. VtR Inc. was among the few companies to win dual awards with two separate systems.
The evaluation focused on the SMART on FHIR international standard framework, promoting "plug-and-play" healthcare applications that enable interoperability between different health information systems without extensive custom integration, effectively reducing redundant tests and improving clinical efficiency. The ceremony also featured an international forum inviting core SMART and FHIR standard developers to share global digital health trends.

Award-Winning System 1: Taiwan Lipid CDS Decision Support System (lipid-cdsu)
lipid-cdsu is Taiwan's first clinical decision support system (CDS) fully developed according to the "2025 Taiwan Lipid Management Expert Consensus." Using the SMART on FHIR international standard, the system retrieves patient LDL-C values and clinical data in real time from the hospital EHR, automatically classifies cardiovascular risk levels, and provides personalized treatment recommendations within seconds.
Core Features
Instant Precision Risk Stratification
Automatically identifies general, high, and very high risk levels with corresponding LDL-C targets (<130 / <100 / <70 mg/dL).
Intuitive Three-Color Visual Alert
Red (severely off target), yellow (not yet at target), and green (on target) status indicators for immediate clarity.
Personalized Medication Guidance
Recommends Statin dose adjustment, Ezetimibe combination, or PCSK9 inhibitor use based on expert consensus guidelines.
Privacy Protection (Zero PII)
No patient data is stored; all data is processed temporarily in the browser only, fully compliant with privacy regulations.
Risk Stratification & Target Values
| Risk Level | Clinical Criteria | LDL-C Target |
|---|---|---|
| General | No major cardiovascular disease, diabetes, or chronic kidney disease | < 130 mg/dL |
| High Risk | Diabetes or multiple risk factors | < 100 mg/dL |
| Very High Risk | Acute myocardial infarction or stage 5 chronic kidney disease | < 70 mg/dL |
Technical Architecture
The system adopts a lightweight front-end/back-end separated architecture, seamlessly integrating with hospital EHR systems via SMART on FHIR (v2.0) OAuth2 authorization. The core data format is FHIR R4, primarily interfacing with Patient and Observation resources, using LOINC code 13457-7 to precisely locate LDL-C lab values. Supports all major browsers and SMART-enabled EHR systems (including Taiwan's MOHW THAS sandbox environment).
Award-Winning System 2: Chronic Prescription Auto-Reporting Assistant (auto-reporting_u)
auto-reporting_u is a FHIR-based smart NHI (National Health Insurance) claims automation solution designed specifically for Taiwan's healthcare institutions, addressing pain points of manual claims entry errors and frequent claim rejections. Through SMART on FHIR technology integrated with MOHW's THAS health archive platform, the system automatically retrieves patient visit data and generates standardized claims data.
Core Features
One-Click Claims Data Generation
Automatically retrieves patient information, visit records, procedures, and orders, converting them into FHIR Claim and ExplanationOfBenefit resources per NHI regulations.
Real-Time Data Validation & Error Alerts
Built-in CQL decision engine validates claims data completeness in real time, reducing rejection rates.
FHIR Bundle Packaging & Upload
Standardizes claims data into FHIR Bundle format for one-click upload to the NHI claims platform.
Eliminate Redundant Data Entry
Frees healthcare staff from tedious administrative claims work, allowing them to focus on patient care — achieving "zero-burden claims."
Technical Architecture
The system integrates with MOHW's THAS health archive platform via SMART on FHIR, standardizing HIS/EMR clinical data into FHIR Resources including Patient, Condition, Observation, and MedicationRequest. Combined with a CQL (Clinical Quality Language) decision engine for NHI rule validation, it automatically generates FHIR Claim JSON and CDS Hooks real-time pre-check results.
Common Technical Standards
Both systems strictly adhere to the following standards, ensuring high compatibility with Taiwan's healthcare information ecosystem:
SMART on FHIR
International standard healthcare application framework enabling plug-and-play EHR integration.
TW Core IG
Compliant with Taiwan MOHW FHIR Interoperability Implementation Guide (Taiwan Core Implementation Guide).
FHIR R4 + CQL
Built on HL7 FHIR R4 data format and Clinical Quality Language decision logic.
THAS Platform Compatible
Fully supports Taiwan Health Archive System (THAS) sandbox and production environments.
This recognition affirms VtR Inc.'s commitment to smart healthcare and demonstrates the company's technical depth in SMART on FHIR application development. VtR Inc. will continue to advance clinical decision support and healthcare process automation, contributing to Taiwan's digital health transformation.
